I am trying to create an account, but it says my date of birth is incorrect

When creating an account, you will be asked to complete identity verification through your date of birth, provide your personal information, and enter your contact details. The identity verification field is determined by your organization.

If you receive an error when trying to verify your identity, reach out to your organization/clinic directly to verify the information, and make sure they have entered the correct date of birth for you. 

You can find your organization’s contact information at the bottom of any invite or reminder emails sent by Thrive Health.
