As a patient, you can download your medical questionnaire once 100% complete. Please follow the steps provided in the video for further guidance or follow the steps below to view and download your questionnaire responses:

Any assigned tasks and educational materials are shown on your account Home page.

Complete the requested questionnaire. After completing all the questions, select the blue Submit button to submit your information to the physician's office.

From the Home page, select View Summary to view and download the pdf summary of your submitted questionnaire.
- A separate tab will open that includes a PDF Summary of your answers to the questionnaire. You can now download the summary to your device or print it.
Note: If you made an error in your questionnaire, please reach out to the doctor's office or clinic directly to which the questionnaire was sent.
