Download Questionnaire Results

Questionnaires are sent to patients through the Thrive Health platform to gather responses on their well-being.

Follow the steps below to download a submitted questionnaire from a patient:

1. Log into your account at https://app.thrive.health 
2. Search for the patient in your worklist and click on the three dots next to the patient's name
3. Click on Patient Details
4. Click the blue Download button next to the questionnaire's name 

You also have the option to view aggregated data for all responses on a particular questionnaire. This feature allows the care team to observe trends across the clinic and provides insight into what treatments are working for patients and where changes need to be made. 

1. Log into your account at https://app.thrive.health 
2. Select your name in the top right-hand corner of the screen
3. Select Organization Settings
4. Select Questionnaires from the left-hand menu 
5. Select the View Aggregate results icon shown below
6. Select the Download icon shown below to download the questionnaire responses to your device
